I acknowledge and understand that there are risks associated with participation in the pickleball tournament. Pickleball is an active sport, and playing may lead to, but is not limited to the following: contusions, fractures, scrapes, cuts, bumps, paralysis, or death.

1.I, for myself and the Participant(s) named, willingly assume the risks associated with participation and accept that there are also risks that may arise due to OTHER PARTICIPANTS which I also willingly assume.

2.I, for myself and the Participant(s) named, agree to hold harmless and indemnify Child Evangelism Fellowship of Indiana, Central Indiana Chapter, its staff, local committee and volunteers from any and all injuries, liabilities, damages, medical expenses, and economic loss from participation and any first aid, medical care, or treatment provided to my child in the event my child is injured or becomes ill while participating.

3.I additionally agree to indemnify Child Evangelism Fellowship of Indiana, Central Indiana Chapter, its staff, local committee and volunteers for any defense costs or expenses arising from any and all claims, injuries, liabilities, or damages arising from participation.

4.I, and the Participant(s) named, are of physical ability to participate in the pickleball tournament.

There will be a separate waiver for Northside Baptist Church to sign at the event.
